Brain Injury Waiver Program
Revitalizing Lives LLC is now accepting clients under the Brain Injury (BI) Waiver Program! We are excited to offer specialized services to individuals with brain injuries to help them regain independence and improve their quality of life.

Program Overview
The Brain Injury (BI) Waiver is a comprehensive program designed for individuals with a medically diagnosed traumatic or acquired brain injury. Our focus is on developing new independent living skills or re-learning those lost due to the injury. The services provided are based on personalized goals created by the participant, their providers, and their Managed Care Organization (MCO). Progress is evaluated every six months to ensure ongoing improvement or transition to other appropriate supports.

Eligibility Criteria​
To qualify for the BI waiver, you must meet the following criteria:

Age: 0 up to and including 64 years old

Residency: Must be a resident of Kansas

Disability Status: Must be determined disabled or have a pending determination by the Social Security Administration

Financial Eligibility: Must meet Medicaid financial eligibility

Medical Diagnosis: Must have a documented medical diagnosis of a traumatic or acquired brain injury (excluding chromosomal or congenital brain injuries), confirmed by a qualified medical provider

Level of Care: Must meet the criteria for placement in a Traumatic Brain Injury Rehabilitation Facility and for ages four years and older, meet the level of care required for hospital placement. For ages 0 to 3 years, current brain injury diagnostic and treatment recommendations from the treating physician must be provided

Rehabilitation Needs: Must have active habilitation/rehabilitation needs for BI therapies

Functional Eligibility: For ages 4 through 64, must meet the level of care criteria based on the state-approved Medicaid Functional Eligibility Instrument (MFEI) completed by the ADRC
Enrollment Process
To apply for the BI waiver:

Contact: Reach out to your local Aging and Disability Resource Center (ADRC).

Assessment: The ADRC will conduct a BI Medicaid Functional Eligibility Instrument assessment to determine functional eligibility.

Eligibility Determination: If functionally eligible, the KanCare Clearinghouse will assess if the participant meets the disability and financial eligibility guidelines.

Submit Attestation: The KDADS ADCSP HCBS BI Attestation Form can be completed by a qualified medical professional and submitted to the ADRC in place of medical records containing your brain injury diagnostic information.
